Carrying a detailed, up-to-date medical history summary is critical when entering a hospital to ensure safety, facilitate accurate diagnosis, and prevent medical errors. This information is especially important for emergency admissions where, if you are unable to communicate, it can be lifesaving.
Emergency room care focuses heavily on immediate, acute symptoms (such as pain, dizziness, or shortness of breath) while other previously diagnosed conditions that may be relevant to treatment remain unknown.
The attending physician simply does not have time to search medical records therefore must rely on clinical observations and what you are able to communicate.
